MINIMUM DOCUMENTS CHECKLIST

  • Complete Member, Income, and Asset Profiles at www.myhousekeys.com  

    • Refer to the video HERE for instructions

  • Income (Will vary depending on your household income sources):

    • Last 30 days’ Paystub(s) for each job

    • The most current benefit award letters (e.g., Social Security, Disability, Unemployment, Pensions, etc.)

    • If Self-Employed, most current Schedule C and a Year-to-Date Profit and Loss 

      • Statement (signed and dated) – for each business separately.

  • Most Recent Filed Federal Tax Return (all pages) – 2025 -- Including W-2s and 1099 forms

  • Most recent 2 months’ statements from all asset accounts (Checking, savings, retirement, etc.)

  • Documentation. Documentation for each of the Household Members (Government-issued photo ID)

  • Upload City Preferences supporting documents to prove you meet the city’s preferences (if applicable). Please refer to the program Exhibit E

  • HOMEOWNERSHIP ONLY: Loan Pre-APPROVAL Letter from the approved HouseKeys Lender List

    • Loan Docs: 1003 and 1008 (from your loan officer)

  • HOMEOWNERSHIP ONLY: Gift Letter, if applicable, and the donor’s current bank statement with sufficient funds

  • RENTAL ONLY: Complete the property rental application. View the opportunity drawing page for the property-specific application.

IMPORTANT

  • In some cases, certain Opportunities require more documents than those listed here. Please double-check the Opportunity Drawing Notice for the property you are applying for to ensure you submit all THE required supporting documents by the designated deadlines.

  • Applicants who miss the deadlines to enter the drawings or who submit the necessary documentation after the deadlines will be skipped.
